New Patient Paperwork

Life and Health Chiropractic and Massage Centre offers our patient form(s) online so they can be completed in the convenience of your own home or office.

The forms will ask for your initial complaint, collect the detail on that complaint, before allowing you to describe additional complaints. If you wish to edit the form after you have completed it, you will be able to do so before you sign the form. Please be sure to continue to scroll down until you have reached the bottom of the form where you will receive instruction as to how to move forward finishing the form.

If you are having difficulty with the form, please give us a call or you can come into the office to complete the form where we can assist you. You may use your laptop, smart phone or tablet to complete the form. After signing the form, the form will be saved to a secure (HIPAA compliant) server for retrieval in our clinic.
